Benefit Checklist

We recommend you follow these steps as part of your evaluation process for benefit election. These steps are not required, but may help you make a more effective decision.

• Review benefit options
• Review benefit comparison sheet
• Review "What plan is right for me?"
• Log into www.mycigna.com
  and review your healthcare expenses for the current year. These can
  help you estimate your potential healthcare expenses for the upcoming
  year. Compare how much your healthcare expenses would cost you under
  each of the 2011 plans.
• Review the 2012 Wellness Incentives
• Fill out the Benefit Election form and any applicable enrollment/change forms. Return completed forms to Human Resources.
• Complete the SJC CIGNA Enrollment Form if you need to make changes in the dependents you cover in your family plan.
