The SJC/CIGNA Incentive Points Wellness Program for 2012

We heard you!

We have expanded and improved our wellness options to fit most busy lifestyles.
We offer flexible programming:

  • On campus fitness classes
  • On campus wellness seminars
  • Online fitness programs
  • Online educational programs
  • Personal Health Advisors and Health Coaches with customized programs

Partner with us to improve overall health on campus!

Seventy percent of diseases are preventable. By partnering together we can change unhealthy behaviors and improve health of on campus. Improving your health is the only sustainable way to reduce health premiums.

SJC/CIGNA Wellness Incentive Web Pages

  1. Log on to myCIGNA.com
  2. Look for the section along the bottom of the screen labeled Earn Points, Get Rewards
  3. Click on Earn Rewards
  4. You are directed to the SJC/CIGNA Incentive Points programming web pages
  5. Begin by completing the My Health Assessment for yourself.
  6. Have your spouse complete the My Health Assessment too.
  7. Choose programs that help you meet your individual health goals.
  8. Earn your maximum 300 points. 1 point = $1 dollar.
  9. All wellness dollars are deposited into your FSA, HRA or HSA Account.

Below are the wellness program options along with completion milestones, time frames, and CIGNA Health Care Points.

Preventive Programs

My Health Assessment - This survey is the first step to understanding your current health
status and on what areas you may need to focus.

Completed once per year.

Mandatory

Preventive Care Office Visits - This includes your annual
visit with your physician as well as some standard screenings that are
recommended. CIGNA provides a form for each of these options that you
can take with you to the appointment and have signed. 

  • Log into the myCIGNA Wellness website described above
  • Click the link for the appropriate milestone and download the provided form
  • Submission instructions are included on each form

Completed once per year.

My Flu Shot - Get your yearly flu shot.  This item can be self reported online.

Dental Care - Complete annual preventive dental care.  Download the form from MYCIGNA to take to your Dental appointment.

Advising/Coaching & Prescription Programs

My Screenings - CIGNA provides the Energy & Performance
on-line health coaching program.  This provides each participant with an
opportunity to enter standard screening values for tracking.  They
include:  BMI, Blood Pressure, Cholesterol, Blood Glucose, Height, and
Waist Circumference.

Completed once per year.

CIGNA Health Advisor - Take advantage of the CIGNA Health and Wellness Advisor for several layers of wellness.

Each completed once per year.

CIGNA Health Management Programs

Program Points

CIGNA Lifestyle Management Programs - CIGNA provides
telephonic lifestyle management programs that include:  tobacco
cessation, weight management and stress management.

Completed once per year.

The options are:

 

Complete three calls with a Lifestyle Management Program Coach.

Complete a Lifestyle Management Program.

Complete a Lifestyle Management Program and achieve  the targeted outcome.

Complete the six-month follow-up call with Lifestyle Management Program Coach and maintain targeted outcome.

CIGNA Weight Management - Weight management is a significant factor in many health issues.  Complete a monthly weight management activity (examples:  Weight Watchers®, Jenny Craig®).  This item can be self-reported on-line. 10 per month

SJC Lifestyle Management Programs - Participate in one of our many on-campus programs.  This item can be self reported online.

Complete two programs per year.

The 2012 Program Options will include:

  • Positive Pounds
  • EveryBody, EveryDay
  • SJC Boot Camp
  • New Programs to be developed based on the SJC Culture Audit
