Saint Joseph’s College continues to partner with CIGNA to offer extensive opportunities to improve your health and earn points. Customized SJC programs and fitness classes are available. Please read through the options and points available to decide which program best fits your needs and lifestyle.

Please participate in the SJC Culture Audit

What is a culture Audit and why are we asking you to complete this?

  • A Culture Audit helps determine the culture that currently exists within our SJC community.  Surveying your opinions will assist the SJC Wellness Team with providing the programming that YOU want to have as you strive towards your individual wellness.  It helps us identify what keeps you motivated and happy and offers information on areas we can make changes or improvements.

As part of the wellness programming your participation in the SJC Culture Audit is encouraged.  Individuals who participate in the survey will be entered into a raffle for a$25.00 gift card to Hannaford.  Individuals must complete the survey no later than Friday, January 31, 2014.

Completing the survey will take only a few moments of your time and we appreciate your honest opinion when answering the survey.  The survey results will be published on the Wellness website once all participants have completed the survey.  No names or personal information will be shared..

Personal Health & Wellness Programs

 
PROGRAMS POINTS
Personal Health Analysis - This survey is the first step to understanding your current health status and what areas you may want to improve.  Completed once per year. Mandatory
Wellness Screening - Complete a personalized biometric health screening.  Screenings include: BMI, Blood Pressure, Cholesterol, Blood Glucose, Height, and Waist Circumference.  (Off-site screenings can be done).  Completed once per year. 25

Achieve Health Goals (listed below)
Each completed once per year.

     Achieve a healthy BMI of less than 30 50
     Achieve a healthy total cholesterol level of less than 139/89 50
     Achieve a healthy total cholesterol level of less than or equal to 239 mg/dl 50
CIGNA Online Health Coaching - Health coaching can significantly improve your chances of making positive health changes and choices. Completed once per year.
Online options are:
 
     Control your weight without dieting 10
     Gain control over stress 10
     Quit smoking or chewing tobacco 10
CIGNA Health Coaching by Phone - Health coaching can significantly improve your chances of making positive health changes and choices. Completed once per year.
Telephone options are:
 
      Achieve a goal to overcome a health problem  50
      Get help improving lifestyle habits 25

Preventive Care Programs

 
PROGRAMS POINTS
Preventive Care - This includes an annual visit with your physician as well as recommended screenings. Completed once per year***.
The options are:
 
     Annual Physical 50
     Mammogram 25
     Cervical Cancer Screening 25
     Colon Cancer Screening 25
     OB/GYN Screening 50
     Flu Shot 10
***Preventive Care options above are now automatically
earned based on claims processing (no form required)
 
Dental Care - Complete annual preventive dental care exam.
Self-reported once per year. (no form required)

Health Management Activity - Participate in a monthly SJC or CIGNA onsite health management activity (self-report once/month at 10 points each for a total of 120 points for the year).
Examples include:

  • Lunch and Learns (these can be CIGNA, SJC or outside presenters)
  • Cooking Series
  • Wellness Seminars
  • Health Fair

Physical or Weight Management Activity - Take part in a monthly physical or weight management activity.  (Self-report once/month at 10 points each for a total of 120 points for the year).
Examples include:

  • Based on successful adherence to CDC guidelines
  • Using the Alfond Center
  • Participating in an adult league (soccer, kickball)
  • Weight Watchers ®
  • Jenny Craig ®

Wellbeing Onsite Program - Complete a quarterly SJC or CIGNA Physical/Mental Wellbeing Onsite Program (Each activity is worth 25 points and may be self-reported up to 4 times for a total of 100 points for the year).
Examples include:

  • Attended 80% of a fitness class
  • SJC(3) Choose, Challenge, Change program
  • Monks 5K training program
  • Mini Tri training program
  • Positive Pounds
  • Just Walk 10,000 Steps a Day
  • Strive For Five
  • Tobacco Cessation Program
